Subject:                     Approve Contract between County of Lake and North Coast Opportunities, Inc. for the Housing and Disability Advocacy Program, in the amount of $167,162 from April 1, 2020 to June 30, 2021, and authorize the Chair to sign.

 

Executive Summary: (include fiscal and staffing impact narrative):

The Housing and Disability Advocacy Program (HDAP) targets individuals who are disabled or likely disabled and who are experiencing homelessness, giving highest priority to chronically homeless individuals or individuals who are homeless and rely most heavily on government funded services. Using the Housing First approach, HDAP aims to assist eligible individuals by finding and maintaining stabilized permanent housing while providing disability income advocacy services. This is a state funded program, which offers more housing opportunities for one of our community’s most at risk populations. The Contractor will guide and assist participants through various disability income application processes, including Social Security, while ensuring participants remain housed throughout the determination process with the ultimate goal of obtaining permanent housing. The funding for this Contract requires a dollar-for-dollar match, meaning total program funds are $334,324, with the majority of the match funds provided by the Contractor. Social Services issued a Request for Proposals (RFP) #20-06, which ran from February 3, 2020 to March 20, 2020. North Coast Opportunities, Inc. was the only entity to submit a proposal.
