Subject:                     (a) Waive the formal bidding process, pursuant to Lake County Code Section 38.2, as it is not in the public interest due to the unique nature of goods or services; and (b) Approve the Agreement between the County of Lake and Care Coordination Systems for the Implementation of Pathways HUB Referral System Software for Fiscal Year 2019-20 through Fiscal Year 2021-22 and authorize the Board Chair to sign the Agreement.

 

Executive Summary: Attached for your approval is the Agreement between the County of Lake and Care Coordination Systems for the Implementation of Pathways HUB Referral System Software for Fiscal Year 2019-20 through Fiscal Year 2021-22. This will support LCBHS’ MHSA Innovation Plan of creating a closed loop referral system between LCBHS and other service providers in the community and amongst themselves as well.  This would mean that when one agency refers a patient/client, it would be done electronically, through the Pathways system, to another community agency. That agency would acknowledge that referral and be able to provide additional information about the services provided.  LCBHS has already piloted a much more limited version of this with success.  The Pathways system, unique in its utilization abilities but matching the goals of the Innovation Project, will provide an even greater tool to provide coordinated care amongst service providers and also allow patients/clients to view their own referrals and progress.  This tool and process would make interagency coordination much more possible, allowing agencies to track progress of their clients/patients, and the use community resources more efficiently.  Community agencies/providers and community members have been meeting in LCBHS’ Innovations Committee, planning this and recognizing this is an innovative tool to better serve our community.  Pathways has a history of providing this service and ability in other communities and will be providing ongoing training and technical support to this project.  Funding would be provided from the dedicated allotment of MHSA Innovations funding and from the state Whole Person Care Pilot Grant received this past year. The Agreement amount of $512,500.00 is for a multi-year agreement for the implementation of and support services associated with the HUB Referral System Software. As this is a unique software system, it is in the public’s best interest to waive the formal bidding process.
