Subject: Approve additional lift gate charges in the amount of $300.00 from the purchase of kitchen equipment from Cook's Supply

Executive Summary: On May 25, 2021, your Board approved the purchase of a Tilting Skillet and Planetary Mixer from Cook's Supply. After the purchase order was issued and prior to the shipment of these items, it was determined that an additional charge of $150.00 per item would be charged due to the lack of a forklift at the jail facility to offload the order.

The department forklift was inspected by an outside contractor and it was determined to be unfit for use. The forklift did not meet minimum requirements of the California Occupational Safety and Health Administration. Staff contacted a forklift repair company to attempt to bring the forklift up to code, however the vendor was unable to find the necessary parts to make the forklift safe. The forklift has been taken out of service and is no longer available for use. As a result, extra expenses have occurred resulting in these charges.
