Subject: Approve Amendment No. 1 to the Agreement Between County of Lake - Lake County Behavioral Health Services as Lead Agency for the Lake County Continuum of Care and Elijah House Foundation for Fiscal Years 2020-21, 2021-22, and 2022-23 and Authorize the Board Chair to Sign.
Executive Summary:
This Amendment corrects the title of the original Agreement to reflect a multi-year Agreement, and also corrects the fiscal provisions as outlined in Exhibit C to reflect a multi-year Agreement and correct the total compensation payable (the original Agreement listed the total compensation for only one year, not the entirety of the contract). Lastly, this Amendment adds additional language around the Contractor’s responsibilities and County responsibilities, as well as Exhibit F which is a floor plan of the shelter facility.
