Subject: Approve Amendment No. 2 to the Agreement Between County of Lake and Willow Glen Care Center to Increase the Contract Maximum to $360,000.00 for FY 2021-22 and Authorize the Board Chair to Sign.
Executive Summary:
Willow Glen Care Center provides Adult Residential Support Services and Specialty Mental Health Services, primarily to individuals on LPS Conservatorship. Lake County Behavioral Health Services, in partnership with the Public Guardian, support individuals on LPS conservatorship and a primary focus is on securing the appropriate placement for individuals to receive the services they need and to facilitate recovery.
We have seen an increase in the need for this placement over the past fiscal year and thus request to increase this agreement by an additional $140,000.00 for a new contract maximum of $360,000.00. This will provide enough funding to support the placement of individuals currently at Willow Glen Care Center through the close of the Fiscal year.
