Memorandum
Date: July 12, 2022
To: The Honorable Lake County Board of Supervisors
From: Jen Baker, Health Services Deputy Director
Subject: Approve Amendment 1 to the Agreement between County of Lake Health Services Department and Lake County Office of Education to Coordinate COVID-19 Vaccination Clinics to Eligible School Aged Children, and Authorize Board Chair to Sign
Executive Summary:
In May of 2021 Health Services and Lake County Office of Education (LCOE) agreed that Health Services would apply for funding offered through the California Department of Public Health (CDPH), under the COVID-19 AB 86 Safe Schools for All Funding initiative, subcontracting with LCOE to complete the grant activities and required reporting.
Lake County was allocated one hundred five thousand dollars ($105,000) under this grant funding.
This Agreement provides funding for LCOE to coordinate, host, and staff school based vaccination clinics throughout Lake County to increase vaccination rates for eligible school aged children.
The Agreement was originally slated to term on June 30, 2022, but CDPH recently announced active agreements many be extended through June 30, 2023, to utilize any remaining funding.
Health Service's requests your Board's approval of this Amendment 1 to extend the Agreement term, allowing the department to continue support of LCOE coordinated COVID-19 vaccination clinics.
