Executive Summary:
On February 2, 2016, the Department of Public Works entered into an Agreement with Drake Haglan and Associates, Inc. for Engineering Services for the Upper Wolf Creek at Wolf Creek Road Bridge Replacement Project in Lake County, CA. The Agreement was modified by Amendment One to allow for an extension of the contract term, and substitution of key personnel required during the design phase of this project. This engineering services contract has been reassigned from Drake Haglan Inc. to Dewberry Inc., as the former corporation has been acquired by the latter.
Amendment Two will add additional engineering design services involved with the relocation of utilities including those of PG&E, AT&T, and water mains belonging to Lake County Special Districts. Amendment Two will provide for the additional effort required for Task 5, "Final Design Engineering", Task 8, "Utility Relocation Coordination", and Task 12, "Water Line Relocation and Design". A fixed fee and other direct costs are included for this additional scope of work. Caltrans has concurred with this amendment and associated costs are eligible for reimbursement. As a result of this amendment, the contract amount will be increased as indicated below by task and by bridge:
